The following proofs are required by Alabama state law for initial enrollment in public schools:
- Certificate of Exemption or Certificate of Immunization against the following diseases:
- Diphtheria, Tetanus, Poliomyelitis, Measles* (Rubella, Rubeola), Mumps, and Whooping Cough (Pertussis). Students enrolling in kindergarten or first grade must have documentation of Varicella (chickenpox) vaccine or physician's documentation that the child has had chickenpox disease. When the immunization certificate expires, the student will have two weeks to present an updated certificate. If an updated certificate is not presented within two weeks, the student will not be allowed to attend school.
